What kind of phone are you using? Was your number used in another phone prior to the one you are using now?
Are you on stc? saudi telecom company? if yes, at the back of your sim bears puk code, you can use it.
or log on to your mystc.com.sa on your browser, if you have the account, once logged in, navigate to your phone number and select the appropriate one, then select sim service, you can find your puk code there.

Here's what it means:
PIN Unblocking Key (PUK) - If a GSM or GAIT subscriber enters
the wrong PIN three times in a row, then the wireless mobile phone is
locked until the correct PUK is entered.
What to do:
Call ATT and ask for the PUK code. You will need to verify the account when you call, so if you do have the info (last 4 of the SSN on the acct or the password)
Other option is to bring that to a cellphone repair shop and have it hack on its PUK .

You need to call MTN Nigeria to unlock the PUK, or simply use a different sim card.
UK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.
The PIN and PUK are related to the SIM card, not to the phone.
If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.
