1. if you've already registered the sim with the network- go to home page on web login to your account click on the help section click on unblock my sim, enter your mobile number & hey presto PUK code appears
2. ring the network & ask them for your PUK code

If you log onto your companys website under your account and go to the option that says devices you can get the puk code- If you enter in too many numbers it will permanently block your sim card and you will have to buy a new sim card from your cell phone company- they are about $20

PUK code can be used 10 times,then SIM card is gone forever. PUK is needed if 3 times wrong PIN was typed. So type PUK and when ask New PIN choose any number (4 digits) then re-enter again and that's it.
