You didn't say what brand or model, so here's a generic answer:
Find the instructions to reset it to factory codes. Sometimes this is as easy as removing the battery and waiting an hour (or a day) and other times you have to enter special codes, or disconnect a jumper, or connect 2 points with a wire to erase the codes. Once you have the lock reset, put brand new batteries in and follow the instructions for programming a brand new lock.

If you don't have the instructions, you can often find them online by entering the brand and model # into the search box of Google or YouTube along with a word or phrase like "instructions" or "manual" or "getting started."

SOURCE: I just installed a Smartcode Deadbolt and I follow

On the back of the unit slide the cover off and push the 2nd switch (on the contorl panel of four switches) down.

To program the code, push the black button once and enter a 4 to 8 digit code, then press the middle lock button. To set a secondary code, press the black button twice, enter 4 to 8 digit code and press the lock button.

This should all be self explanitory in the instruction manual.

I have one of these on my front door and its fantastic. I carry a key but have not used it in months. Bravo Kwikset, Bravo!

SOURCE: kwikset smart code touchpad electronic deadbolt

I have the same problem, electronic locking system went dead but still beeps, stuck using keys now...according to the kwikset website though it says "Warranties cover the original residential user of Kwikset Products against defects in material and workmanship as long as the original user occupies the residential premises upon which the product was originally installed." so hopefully they will fix the problem

SOURCE: kwikset smartcode deadbolt newly installed unlocks when lock button pushed. I have taken batteries out and reset few times with no success.

"I have installed your Powerbolt, but when I push the Lock Button, it unlocks? What should I do?"
First remove the cover on the inside of the Powerbolt unit. On the right hand side under the batteries, you will notice that is says "Door Direction Jumper". If the cap is mounted on Prongs 1 & 2, carefully remove it and move it over so that it mounts Prongs 2 & 3. A is right handed, B is left handed.
These instructions will work just fine for you:
http://www.kwikset.com/Customer-Support/Product-FAQs/Faq/50160000000HhhEAAS.aspx

Notice that the above is a .PDF file type. That means that you need to have Adobe Reader or similar installed on your computer to view it. So if you are unable to view the file, download (for free) and install Adobe Reader first. You can get it here:

http://get.adobe.com/reader/

These locks are made to work on a door which, when you stand outside and face it, has its hinges on either the left-hand or right-hand side of the door (= door "handedness").
The jumper controls that for the appropriate installation. Yours is set wrong!

I just bought your defiant electronic deadbolt. It works backwards ... Lock is unlock and unlock is lock ... I've been working this for 7 hours!

I would recommend taking the deadbolt back to wherever you purchased it from and exchanging it for a new one. It sounds like you got a defective unit.

Just so there’s no confusion, this website is a general question-and-answer site for anyone who needs help fixing something. It’s not the customer service website for the manufacturer or retailer you got your deadbolt from. I do not represent the manufacturer or the retailer. I’m just a random guy on the internet trying to help out. :)

I lost the factory code and the deadbolt won't unlock with our programmed code. We did the reset but now can't do anything because we don't know the factory code. What do we do?

Hi, Bailey,
I am assuming by your photo that this lock is electronic. In my experience, most of these locks, when reset will default to manufacturer's or engineer's code. This is often as simple as 0000, 1234, 9999, etc. Try this or a number of simple combinations. If this does not work, try the manufacturer's website under SUPPORT and follow links to download or request a user manual or installation guide.

POWERBOLT Weiser Powerbolt Deadbolt, not opening. even after changing batteries

You didn't say much about the lock's symptoms beyond it worked great, then stopped. Here is the link to the troubleshooting instructions. The Weiser brand and Kwikset brand Powerbolt are identical (Black & Decker), so these instructions will work just fine for you:

http://www.kwikset.com/Customer-Support/Product-FAQs/Faq/50160000000HhhEAAS.aspx

A note about PDF files: If you cannot view a given file, you need to first install Adobe Reader. You can get the free download here:
http://get.adobe.com/reader/

Weiser has a one year warranty against electronic malfunction and a lifetime warranty against mechanical and finish problems.
You can review it and contact them for further assistance from this link:

http://www.weiserlock.com/CustomerService/WarrantyInfo/Powerbolt.aspx

Since you've had your lock "working great for a long time", I'll assume that it's probably an electronic problem, out of warranty, and Weiser will want you to order a replacement power board at a considerable cost. You can try leveraging them with Dissatisfied Customer outrage, and maybe they'll send you the part for free. Then, it'll work great for a long time, and you will be back in this same boat again.
After more than a year on this website, I've seen far more Powerbolt problems than any other, which leads me to my opinion that it is not very reliable, to say the least. After contacting Customer Service, if they won't replace your part for free, you may wish to consider cutting your loss and installing another brand of lock. Again, it's only my opinion that you consider a Schlage lock instead.
Good luck!

When I install new batteries it won't stop 'squealing'...what could be the problem? Ugh - I can't even use it like that!! thanks!

It was a Garrison - I've since learned that the problem was it was screwed a little too tight and was pushing against the circuit board. I took the back off (where the batteries are) and loosened the screws a little and the squealing stopped!

The lock is beeping. I put new batteries in and reset the code and it still beeps.

Here is a link to the installation and programming instructions for your lock:

Starting with Step 7 (battery replacement), it is very important that you follow the procedure very carefully throughout the rest of the steps.
If your problem persists, there is contact information at the bottom at the instructions sheet to enable you to pursue parts and further technical advice directly from Kwikset.

Good luck!

Door opens half way then closes quickly with electric operation sprayed wd40 on rollers but still not working properly

Sounds like the 9 volt battery is very low, please put in a new, fresh 9 volt battery and see if that helps, as you've already addressed any issues with sticking, etc. When the battery(ies) are low, some electronic locks don't have enough power to fully pull in the dead bolt and the internal spring can overpower the small motor and cause the lock to re-engage quickly.

If you're getting a flashing red light when you enter in your key code, followed by flashing green light, your battery is low, if you're getting a flashing red light, the batteri is critically low.

If you have recently installed this lockset, it is possible that the alignment is off and is causing the deadbolt to drag and is preventing free movement. You'll need to adjust the lockset to allow the deadbolt to move freely by turning the hand knob with the door shut all the way.

If that doesn't help, please contact Schlage directly by calling them at the numbers provided:
