UGH I had such a hard time with this, because it isn't clearly explained.
So here is a solution that should work (it worked for me!):
1. Go to Settings, Bluetooth, and Go to the "Paired Device" menu and leave it open.When it asks to turn BT on, you can do so.
2. This is the part here that screws everyone up. Take the headset and press and hold the multifunction button. KEEP IT HELD DOWN! The light should turn on and stay on. While you are holding it, open the microphone. The light should have been on while you were doing this and still be on. If it didn't stay on the whole way try again from when the mic is closed.
3. Remember the "paired device" menu? Select "New" and it will auto search for the headset.
4. It should be found quickly. The headset (that you should still be holding down but with mic open) will blink, then your phone will ask if you want to pair with Motorola HS850. Say ok, then it will ask for passcode 0000.
5. The headset will blink and the phone will say it was successful, and ask if you want it to be turned on. Say yes and make a test call. It should be fine from there.

Circuit/System Description
The Bluetooth ® antenna is a small fixed antenna connected directly to the telematics communication interface control module and is used to send and receive signals from a Bluetooth ® enabled cellular phone. The antenna utilizes no cabling and is not external to the vehicle.
In order to use hands-free calling, the cellular phone must be paired to the vehicle. Up to five devices can be paired to the vehicle at one time, but only one can be connected at any given time. To pair a phone, the customer must know how to operate the Bluetooth ® functionality of their phone. The pairing process must only be done one time for each phone, unless that phone’s information is deleted. For safety reasons, the pairing process is disabled while the vehicle is moving.
Diagnostic Aids

The purpose of this diagnostic is to verify the ability of the telematics communication interface control module to pair to a Bluetooth ® device.

Before performing this test, verify compatibility of the cellular phone(s) the customer is attempting to use with the vehicle. Based on the cellular phone’s service provider and the manufacturer’s implementation of Bluetooth ®, not all phones support all available Bluetooth ® functionality. A vehicle and feature compatibility list will be provided via the GM Bluetooth ® website: http://www.gm.com/vc/bluetooth/

If the vehicle passes the following tests and a compatible device is being used, the concern may be due to a device malfunction or an incomplete/improper pairing attempt.

A Bluetooth ® test tool or equivalent can also be used to verify the ability of the customers cellular phone(s) to pair with another device.

Noise on the same frequency (channel) as your bluetooth connection can inter fear with the connection process.
Check the IOS version you are running. Update if needed via WiFi.
Try outside, away from WIFI & microwave ovens & hotspots & other smart phones & devices in your area.
Turn off your Bluetooth on the iPhone.
Reset the iPhone by quitting all Apps then Hold down Top & Round button together for 5 seconds or more.
Restart your phone with top button (3-4 seconds)
Erase the old bluetooth connection name and restart the discovery routine to establish a new connection.
Last Resort..... Do a full restore or the iPhones IOS
If all else fails.... drop in to your Apple Store

Pairing Instructions
1. When you turn on your Samsung Bluetooth
headset for the first time, it proceeds into pairing
mode immediately.
2. If you have paired the headset with other
Bluetooth devices, make sure that the headset
is off. Press and hold the Multi-Function button
until the blue indicator light stays solid blue after
blinking.
3. Set your Bluetooth phone to discover the headset
by following your phone's guide. Typically, the
steps involve going to a 'Setting', 'Connection',
or 'Bluetooth' menu on your phone and then
selecting the option to discover Bluetooth devices.
4. Your phone will find the Samsung headset and
ask if you want to pair with it. Confirm this by
pressing the Yes or OK key.
5. Enter a passkey or PIN, 0000 (4 zeros), then
press the Yes or OK key.
6. If pairing was successful, the indicator light will
flash in blue 10 times rapidly before going to
flashing every 3 seconds. If unsuccessful, the
light remains on, and you will need to re-attempt
pairing.
7. When you turn on your headset after pairing
with the phone, the headset is automatically
connected to the phone.
8. When you turn on your phone after pairing with
the headset, the phone is connected to the
headset by pressing the Multi-Function button.
