Certainly not an unheard of event. In fact, so common that if you search for "stuck sim card in (fill in your device brand and maybe model)" you may pull up a video or instructions for your particular device. And you can even search for "sim card stuck in wrong slot." Acuatually, I would not be surprised to find "sim card stuck stuck in ear" for about any conceivable thing that can happen to a sim card has happened. That said, here are some videos:

To clean your sim card, remove the SIM card from the phone. Rub a dense white eraser along the exposed metal part of the SIM card. Make sure that the eraser is brand new, otherwise you could get the SIM card even dirtier.
Remove the back of your phone and the SIM card. Spray canned air into the back of the phone, removing all dust and dirt particles, especially those that could be stuck in the SIM card groove.

More specifics would help get a better answer.
If this is in a cell phone, the SIM card generally has a small tab to depress that ejects it, or sometimes a slot you hook with a fingernail to pull it out.
A couple times I mistakenly put the memory card in the SIM card or vice versa and when I have done that I get a safety pin and carefully fish it out from the corner that shows until I can get hold of the errant card and gently pull it straight out.
