If it's really worn you'll probably also need to replace the cogs and possibly the chainrings. Old drive train parts don't mesh well with new ones.

The crankset and pedals on that bike or any other are not unique to one model. Either the original or compatible replacements are available at a variety of sources, but there are several dimensions that are important.
