Select Brands, the maker of Kitchen Selectives, doesn't list your model. This might mean it's no longer in production or was a custom model for a retail outlet. This page at their site gives contact information. All I can suggest is that you contact them and see if they can tell you where you can find an original replacement, or see if one of the universal replacement carafes at retail stores might be a close enough. A box on ebay says: Put bottle inside, add ice to neck of bottle, add water, turn on and set timer for 10-20 minutes. It chills from 77* to 55*or 33* (can't read numbers clearly) in 10 minutes. Can also be used on other drinks and canned drinks. Can be used as an ice bucket as well to keep the opened bottle chilled.
I just bought one at Goodwill for $4 today and was looking for the instructions as well. Hope this helps!
