A PUK (Personal Unlock Key) code is provided by your cellular network operator, should be on the same paperwork (starter pack) as your PIN code. It's related to the SIM card, not to the phone.

It is used to unlock a SIM card in case the PIN has been entered wrong 3 times. The phone then gives you 10 chances to enter the PUK code. After successful PUK entry it allows you to set a new PIN code.

Some operators issue only the PUK number and no predefined PIN, so you can set your own PIN from the start.

Entering a wrong PUK code 10 times locks the SIM card completely and irreversibly, and all the contents of the SIM (phone numbers and SMS messages stored on it) are lost forever.

If you can't find the starter pack with your PUK code, you'll need to contact the mobile operator's hotline to get it.
