Need to format.but it showing security code which is not 12345

SOURCE: i cannot understand why my phone does not accept the default security code 12345

the phone does not accept even the default security code.
i had changed it once but then again changed it to 12345 .but now it is not accepting.
I would like to format it but can i know how to do it?

This works on most of the Nokia phones.
