1. Download this tool:

2. Format

(Before removing USB flash drive click on the Safely Remove
Hardware icon and then click on USB flash drive you want to remove)

Does it work in some other USB port?

Does it work in some USB port on a different computer?

If not, then take a sledge-hammer to it to block ALL future attempts to recover any data from it.

Then, spend $8 to buy a replacement.
