did you mean log out from all server or deactivate your account??

and deactivate account, you can go to settings on facebook, then go to security settings. you will see "Deactivate your account" at the bottom. follow the steps and your account will be deactivate, your account also will stop receive notifications. but once you log in again, your account will activate automatically.

Then you've been hacked, and the accounts are toast. Get new accounts, use secure passwords, and tell everyone the new accounts. Use a password manager like Lastpass.
Wait a few minutes, ideally about an hour, clear your cookies on your browser even if you are logging in from a tablet, phone, or pc browser.
if possible, delete the app and reinstall it after you change your passcode.
try logging in again, this time click send only once and wait up to 10 mins for the code to arrive.
When it does, use it and click OK only once, don't reload or keep hitting send or OK repetitively as it makes your browser resend all information all over again.
Well . . you should try to clear browsing data of your browser by deleting cookies . . and then try logging again . . if you don't remember your password . . then . . click on forgot password and try answering security question . . which must be known to you . . and thus can get to open your account again . .
If this fails . . wait for 24 hours and then try logging again . .
If it de-activated, then go to yahoo mail,give ur e-mail address when ask for activation.Are you getting de activation message? if so, then easy to activate by following the instruction.
If you have get the message that "your password has been changed" then it is a perfect hacking.

can u still remember any important events in your life? try your birthday or someone special to you. Also try again sending some request. Just click forgot password then writer your log in email you used in facebook.
Boot into safe mode (press F8 key several times just before Windows boots up).
Select SAFE MODE
Wait for it to boot, and if there is an Administrator account, choose it. More times than not, there is no password on this account. From there you will be able to change YOUR account password via User Accounts under CONTROL PANEL.
