It is the hardware, the earpiece, you answered the wuestion yourself.
When there is no sound thru the earpiece, try using the speake mode or a bluetooth, if both works, then it's the earpice that is damage. I have no idea what phone model you have, but it can be replace or fix.
I've read quite a few posts from people complaining about the H500's earpiece being too loose and it flopping when they turn their heads. Here is a fix. After wearing mine for two days I was aggravated enough to try and find a solution. I love the H500 and didnt want to give it up,but the dang thing WAS loose... so :
1. Take the earpiece off just as if you were going to make it as if you would use it on the opposite ear.
2. Notice the sharp bend at the top of earpiece.That is where we will make the adjustment.
3. Get a candle or have a friend to hold a lighter steadily for you
4. Run sharp bend of earpiece thru the flame (DO NOT HOLD OVER THE FLAME) paying attention to the plastic,when it takes on an oily smooth look it will be warm enough to GENTLY push the two ends towards each other to tighten the hook shape. Run the earpiece under cool water when desired shape is reached and make sure plastic is cool. reattach the earpiece and place on ear.
5. Shake head side to side and that baby "shouldn't" budge at all. if it does adjust a little more using the above steps.
I hope this helps as it was one of the biggest gripes out there.
Use a handsfree to confirm once again that the EARPIECE is faulty. If you are able to open and check the earpiece then remove and check the sensitivity using a AA 1.5 Volt cell. If the noise is not heard well then the earpiece must be replaced. Also check for blocked ports of the earpiece panel /dust filter which can reduce the sound.

Tray to open up your phone also open all screws on it,than check the earpiece that places middle up the phone screen.Usually the problem come from an earpiece connector are not connecting well to VCB.I hope it will solve your probem but if not,it mean the earpice was broken and need to change.Good luck..
This is not a setting problem, the problem is that the earpiece of your phone is bad that is why you cannot hear callers voice.
The speaker you put it is for the ringer, it does not have anything to do with earpiece and you can hear your caller through it.
To fix your earpiece problem because you cannot be putting your phone on speakers always because atimes you will want privacy.
To fix the earpiece problem, you will need to replace the earpiece on your phone in other for you to hear your caller. just change the earpiece as advice and you will see that the problem is solve.
The problem is on the earpiece part. Sometimes, it could be fixed by cleaning the metal connector on the earpiece area (if you could dismantle the phone). Or if not, the earpiece should be replaced. I already fixed a bunch of those phones.

That would depend on, among other things, how broken it is.
