Take card from the phone. Wipe the SD card . Switch the phone off.
Replace the SD Card. Turn phone on and it should find it automatically.

I myself would replace that battery, yes. If it gets too over-heated, there is a possibility it could explode or catch fire. At the least over-heating can ruin your phone itself.
