i was told there's such a problem in the iPhone 3GS, but I think the best for you will be sending the unit to an Apple service center for replacement.

Could be that it's just a software hickup: press menu and on/off button togetherfor 5 sec. If it's the same after the restart you need a new earspeaker. You can also use a headset, if it works with the headset, you definitely need a new earspeaker.There is a step by step guide too that explains a little bit at eachsteps what you have to do : iPhone speaker repair. You can buy the touch screens online too. There are also iphone repair companies that do it for you.
