That is a document feed or scanner fault. Often times it is simply that molex connector working loose. It is the connector mounted vertically next to the main power cord on the right rear side of the machine. That bottom screw can be dificult to tighten. What I would try first is to shut the machine off, unscrew and disconnect that conntecot totally then firmly slide it in and get both the top and bottom screwss tight. Like I said, the bottom thumb screw can be hard to tighten even with a screw driver because it is a pretty soft rubber the tighteners are made out of.
If that doesnt work. go to the back and disconnect the document feed to scanner cables and reconnect. Other than that, you pretty much have to replace the feeder/scaner assembly since very very few parts are spared for that .

Recently I had to configure a 17" Aluminum PowerBook and a Xerox Workcentre PE 16. That multifunction laser monochrome printer is not supported by MacOS X, but I tried all the same.... and no glue for that, until I went on the Samsung webpage and noticed their SCX-4216F printer. This machine is the same as the PE 16.
So I downloaded the lastest drivers from Samsung's download centre (drivers for Tiger too), installed the drivers on the PowerBook (running 10.3.4), connected the printer and when I selected the printer ... tada! It automatically selected the Samsung driver, so I clicked on OK and started printing. Everything was OK!
But it doesn't end here: even the Lexmark X215 MPF has the same chassis as the Xerox and Samsung. Yet, on Lexmark's website there are no Mac OS X drivers, but Samsung's drivers work just the same. I hope this hint will help others...

System error --> software inside the printer has detected some sort of "software" error.
Cycle power -> disconnect the AC power.
Wait a few seconds.
Reconnect the AC power.
Turn the printer on.

You can try this driver. It may be quirky at first but it should work for basic printing needs. There is a PCL6 and post script try both sometimes it will take one but not the other. Its vista flip a coin :) -Phil
