Contact www.amazon.com Next to the search bar you'll see Your Account. click, choose Your account from the list and click. Next page, far right column you will find help desk options. follow that to leave a message for them. They can send an email with instructions or call you back and walk you through it.

Kindle Fire 2011 1st generation, but to double check it open the settings and see if it runs 2.3 or 4.0 operating system. If 2.3 it's a first generation.
Step 1
Press the "Power" key to wake the Kindle from Sleep mode. The Password prompt displays.
Step 2
Type your best guess at the password into the Password input box. Retype password guesses a total of four times. After the fourth incorrect password entry, the Reset dialog box opens asking if you would like to reset the Kindle Fire.
Step 3
Tap "OK Reset" to reset the device to original factory settings.
Connect the Kindle Fire to a wireless network and re-register the device with your Amazon account.

2. Select the "Manage Your Devices" link
3. Select "Deregister" to deregister your device from your account

I don't think its your charger. The charger port can get loose after time. This is a known issue with Kindle fire 1st generations. Kindle fire 2nd generations have issues with the battery itself. But the issue with the Kindle fire 1st generation is not the ac adapter. The micro usb port attached to the kindle breaks or becomes loose. Bad soldering is the issue.
