Turn off the water supplying the faucet. Now open the faucet. Remove handle by taking off index button and removing screw. You may need to purchase a handle puller, 10-15 dollars, and a stem wrench set. After removing handle, place appropiate sized wrench on stem and turn counter clockwise. Before installing stem, make sure it is in the open position. You may need to use the handle to turn the new stem. Screw in clockwise.

Assemble the hardest part first, but don't tighten it up. Connect the other parts, but don't tighten. Go back to your hardest piece and tighten. Then tighten the remainder.

I would try to find a 3/8 to 1/2 NPT adaptor on Amazon.
If Danco has a website, it's likely that the instructions for your model are available in the support section of the website.

Where does washer go for 12h-6h/c stem from Danco for faucet?

Is it brass, stainless, plastic or rubber? I think a stainless one might be for the knob screw, I don't think there is one inside, it might just have fallen into the pack. We get weird stuff all the time that doesn't belong. Hope this helps.

Looking for cartridge for Tuscany Brass Tub Faucet Single Handle appears to be Ceramic Disk Type

While there are several cartridges that look similiar to what you have posted, Danco it self is an aftermarket parts replacement business. From my point of observation, your posted pic is of a Delta stem which visually has some distinctive attributes. Most every thing Danco makes is a copy or OEM replacement part packaged under their name.
