On the rear of the deck, usually under the black plastic shield. Lift the shield (like you were putting on the bag) you should find a white sticker with black lettering. The model number is 5 digits. If it is a commercial type model, sometimes they hide the sticker behind the handle name plate.

im no expert but had a similar problem with an old craftsman mower. the belt keeper which is an L shaped piece of hardware located at the back of the deck next to the idler pulley spindal. Also could be the deck is cockeyed and needs to be leveled or the wrong v-belt.
Simple rule. V of the belt goes in V pulleys The back of the belt goes on flat or idler pulleys . It may take several tries but I am sure you can figure it out.
