This will clear out user settings, profiles, etc from the phone. (This will delete all user files. Backup anything you want to save to another device. SD card, laptop, Dropbox.. etc.)
Go to the Main Menu
Select Settings
Select Reset Settings
Select Master Reset
A prompt All settings reset to default. Continue? will come up
Scroll to Yes then press OK
Enter 0000 for the Security Code then press OK
Note that if the default Security Code has been changed, enter the new code.
The Resetting Progress will go through to 100% and the phone will be Master Reset
You will then begin setting-up your device again.. email, preferences, apps.. etc.

You may find other options do exist such as the power button while
pressing the space key and delete key, and caps/ shift button will also hard reset the phone.
It all depends on whether or not the *# keys work. Sometimes they won't work if the phone is in phone lock or key pad lock and you need to get it open first.
I'm sorry, but I think you will need to contact T-Mobile for support. Call them from another phone at 1-800-T-Mobile. If you don't care much about the info on the phone, then do a hard reset to factory defaults.
