That is caused 99 percent of the time by the fuser.
There is a solid aluminum roll and a silicon rubber ( pressure) roller in the fuser. The pressure roller either developes some flat spots from park ( non running) time and over time heat and use cause it to deform and become out of round. What then happens is when the paper enters the fuser it does so with a slight twist, and the pressure of the fuser, for lack of a better term, Irons in that crease during the fusing process.

Hi, open the right side as if you were removing a jam. You will see 4 silver phillips screws. Remove them, unplug the fuser and slide it out. I think you will find either a paper folded up in the fuser or in the paper path just past the fuser.
The fuser itself probably needs rebuilt or replaced.

Hello,
try to clean paper feeding rollers and chek fuser paper sensor, maybe paper comming too late...chek this just to be sure all its OK whit paper path and fuser.
...or replace fuser module!

try cleaning the microswitches. it may also have a sensor and if the little plastic piece that crosses between them has broken off it will throw codes. mine is ok so i cant say for sure but you can try to glue a suitable piece in there to trick the printer if thats the case or bypass the sensor. just a few suggestions from my experience with electronics
sensor is kind of this: gap is where the plastic part is supposed to go in between.
