You should have got a puk code with your phone.
If not contact your provider and they will unlock it for you.

Well there is no universal PUK code available which you punch in and get the phone unlocked. So do not try to punch in PUK codes which your are not sure of in an effort to unlock the phone, because putting the wrong PUK code, ten times in a row will permanently deactivate your SIM card.
