For only one number on my phone, the contact name does not show up on either caller ID or text messages. Rather, it shows up as either a rectangle or a menu arrow. I have tried deleting the contact and re-adding it. I have also deleted the contact and re-added it under a new name, but it seems that it is somehow connected to the contact number. How can this be fixed? Do I need to reset my phone?
If only one number is not showing on your screen it means that you have blocked it with your application or messaging system that you'll need to take the number out of the blocking list. But if this doesn't help you the number itself might have a privacy feature that you cannot see the number itself but just showing private number.

I think, your contact lists were stored on the sim, just move the contact lists from sim to phone and then try... it will fix the issue.

This is normal. T-Mobile does this. Unless the person is in your contact list it only says the number. There is no solution, because that is not a problem. They did this on purpose, I do not know why. But this is how it is.

When saving a contact to your phone book for some reason you have to save the number with a "1" in front of the area code and number or it will not register as the contact calling. I would save a number and have them call or send text but it just kept coming up as the number itself and not their name. I changed the number with the 1 in front of it and it worked just fine.
