I don't have a way of increasing the volume on the videos playing and the ringer icon does not show the volume level.
I can safely say you connected your iPhone to a speaker dock sometime in the past.
When connecting an iPhone to a speaker dock, the volume control is set through the speaker and not through the iPhone.
Sometimes after unplugging the iPhone, the iPhone keeps thinking that it's connected and won't let you change the volume through the iPhone.
To fix this - simply plug your iPhone to a speaker dock again and unplug it until you can manage the volume with the iPhone.

Is this for a specific ringtone or all ringtones on the phones?
Certain versions of the iPhone firmware have an issue where the volume keeps decreasing. Try this to fix -
1. Press the sleep button on top of the phone
2. Now press the home or sleep button so you see the "lock screen (the screen with the wallpaper that has the unlock slider)
3. Double click the home button and the ipod controls should come up on the screen.
4. The volume is probably set at the half way mark...set the volume to full.

I would try a system restore, plug your iPhone into iTunes and click restore, it will delete everything but once its done it will restore it to when your last sync was.
