Take off the battery and reinsert. Then hold the 2 soft keys (the 2 select buttons at the buttom left and right of the screen) and then press the power button and hold until you see the phone asking if you like to format the phone. Press send (green button).

The T-Mobile Dash™ includes a button sequence which can be utilized to perform a hard reset. This method is much more difficult to do but it will perform the reset if the menu options are not available.

Note: Before attempting this procedure make sure to backup your data using ActiveSync. All unsaved data will be lost.

Hard Reset
1. Power the device off by pressing and holding the Power key.

Note: If device is unresponsive remove the battery.

2. With the phone off press and hold both soft keys (The two keys directly under the screen, used to open menus.)
3. While continuing to hold the soft keys, press and hold the Power button for 1-2 seconds.
4. Continue holding the soft keys until you see a message that starts with This operation will delete….
5. Release the soft keys.

Note: If you see the standard boot up screen load steps 2 and 3 were not performed correctly.

6. A boot splash screen will appear, followed by a black screen prompting: Press Send to restore factory defaults, Other keys to quit
7. Press the Send key.
8. When prompted to powercycle device, select Ok

Note: Once the hard reset is performed it could take up to a few minutes to completely boot back up.

First of all I think you are actually rebooting into the wrong place being either the bootloader or recovery where you need to go, I think that must be fast boot but I'm not very familiar with that model. You need to boot into recovery and first wipe your cache partition let it boot up then shut it back down, reboot back into recovery and select wipe data. Now the important part is to figure out what combination of buttons works for your model and you had to do it just right or you will boot into one of the wrong partitions and end up where you are at. So Google your exact model down to the very last fine print detail and try to find the exact method to boot into recovery and we can go from there. You can just go into settings and factory reset from backup and restore but that isn't as thorough as this which will completely wipe all the residual junk

Security code bypass

it will wipe your phone. well everythin but whats on your sd and sim. most htcs. hold the vol. down button then the power and keep holding. it will come to a boot screan. pick factory reset then when its done. reboot the phone. some you have to hold both vol. in at the same time as the power.

My t-mobil dash does not re-start properly and frezes

The T-mobile dash is know for doing stuff like this. Have you tried doing the Master reset. Here is a guide i've used on mine.

Mine has frozen and constantly rebooted. Both problems were solved with the master reset.

Lost unlock code

Depending on how many modifications, hacks, or customizations that you perform on your Motorola RAZR V3 phone, there may be situations where the phone becomes unusable or completely locks up. If this is you situation, then you may be interested in performing a master reset (or “hard reset”) on your phone. The master reset will effectively restore your phone back to the factory condition, inclusive of all settings. Be warned, any data that you have not backed up, photos you have not transferred, calendar settings, or messages you have not saved will be lost! Make sure you do this first, if possible!

Here is how you perform a Master Reset on the Motorola RAZR V3:
1. Open your phone up and go to “Settings”
2. Next go to the selection for “Initial Setup”
3. Next go to the selection for “Master Clear”
4. Then select to confirm the option.
5. Now enter the security code which should be “000000”
6. Finally press select or ok and the phone will reset itself to the factory condition

Congratulation you have performed a “hard reset”, “hard reboot”, or “master reset” on your Motorola V3 RAZR and it should return to normal operating mode!

They most likely did a master reset. Settings > (General) > Master Reset.
