A Personal Unblocking Code (PUC) or Personal Unblocking Key (PUK) is used in GSM mobile phones and some smartcards to unblock a blocked card.

Most mobile telephones offer the feature of personal identification number (PIN) protection. After switching on the phone, if the PIN security function is not switched off, the user is requested to enter a 4-8 digit PIN enabling the phone's non-emergency calling functions. If the wrong PIN is typed in more than three times, either the SIM card, the device or both become locked. They can be reverted to their original unlocked state by entering a PUC, provided by the service operator through verification. If the wrong PUC is entered ten times in a row, the device will become permanently blocked and unrecoverable, requiring a new SIM card. Cellular phone users are therefore advised by most providers to keep their PUC written down in a safe place separate from the device.

Go to the SETTINGS OPTION in the menu there you will have an option to restore to factory settings. You can use that and restore. Might be it is asking for a pin code when you do it. For default PIN code check your user manual.It must be most probably 0000

your puk code will have come with your sim card.
if you have lost it then only your network provder. i.e. orange,virgin, e.t.c. can supply you this

PUK code in PIN UNLOCKING CODE, which is required when you give PIN number wrongly for 3 times,
This code is OPERATOR specific and not phone specific and you have to call you operator and get PUK if PIN is blocked...

if you provide PUK code wrongly for 10 time, you sim may become useless, then you have to buy new SIM

This has alos happened to me, I put in the wrong pin 3 times, then as it was laste and it was asking for a puk, I turned the phone off to save the battery, when I turned it back on it came up 'Invalid USIM card' is there anything that can be done?

Only your service provider of your sim card can unlock your puk, it's free once you validate you're the owner

Throw it in the bath then turn it back on and it will say sim unlocked trust me it works!

I recently bought a similar adapter for my trax and had the same problem. Taking the plastic cover off of the adapter, I found the following on the back of the 18 pin connecter: a jumper from pin1 to pin 11, a jumper from pin 9 to pin 10. audio l and r wires to pins 14 and 15, common audio wire to pin 18 and unconnected pin at pin17. By accident, I found that if the unconnected pin 17 bent over to touch the wire to pin 14, the headphones started working. So, solder a wire to connect pin 14 to 17 reinsert these pins into the correct slots and you have a working adapter.
