There is normally three reasons:
1. The album you are trying to copy is connected to iCloud Music Library. If you see a Disconnected icon in the upper-right corner of iTunes, you’re not connected to iCloud Music Library. Click the icon to reconnect.
The song isn’t in Apple Music or isn’t available in your current country or region
2. You have Restrictions turned on for explicit lyrics
There are changes or edits to an album.
Delete that grayed out album from your library and add it back from the Apple Music catalog

Try connecting iPhone to USB port & open iTunes.
When iPhone appears under devices, highlight iPhone.
Select Summary/Options and check Manually manage music and videos.
Can then transfer songs from iTunes music library to iPhone.
Only way to transfer songs from iPhone to computer is in iTunes File Menu/ Transfer purchases from Iphone, which of course only works with songs purchased from iTunes store.
Other option is to use Sync which offers options All songs and playlists or Selected playlists
Connect to itunes, it will detect as Ipod, then press sync button
What I did is I made my own playlist of songs for only my iPhone to hold because I don't want all my songs in my library on my iPhone. The I plug in my phone and check the playlist I want to upload to my iPhone then I click sync.
You have to set what songs you want to transfer, or set the iphone (in tunes) to manually manage music.
Use itunes if itunes doesnt have your current songs that are on your iphone then you will need to back them up (otherwise you will loose them) and then add them into itunes. xillsoft ipod rip or similar will take care of this
You create the playlist in ITunes, then select it in your IPhone using the tab music.
You need to create a playlist for the iPhone. This isn't intuitive and I don't know why it's this way. It works great, there just isn't anything explaining what to do. This seems to snag Windows users the most. Maybe Mac users are already used to doing things this way? I don't know.
Below is a picture of my iTunes. Look down in the lower-left corner and see that my playlist is highlighted. At the very bottom-left corner you can click the plus sign to create a new playlist. Then drag all of the music you want to the playlist. Finally, in the music tab of your iPhone, make sure you click the playlist you just made to sync all music on that list to your phone.

I just had this problem and it turned out to be... It didn't like my iPod cable. Switched to the iPhone cable and it worked.
Seems to be an iPhone 2.0 prob, or 3g prob, since I was using the iPod cable to sync my old iPhone just fine.
