This may be caused by loose connections at the foot control jack. It may need to be resoldered or replaced.

The unit will need service. There is a sensor that detects that the tape is turning and has gone bad.

SOURCE: How to erase data from the cassette tape

You are able to erase tapes on this machine by holding down the play and rewind buttons together. To clear the entire cassette, you'll need to physically turn the tape over and repeat.

Hope that helps

I have a 720 Transcription System which I can't erase any data off. There is a padlock symbol on the LED display. How do I get rid of it?

There is a tiny switch in the mechanism that detects if the erase prevention tab is broken off the cassette. Try another tape to see if it is the tape, otherwise, that switch is probably broken. The tab is located to the right of the orange colored area on the picture. There is one one each side of the tape.
