SOURCE: How do I replace the toilet seat on a Kohler
I repaied one of these laast week and it had the Plastic nuts in between the webbing on the seat where the lid connects I believe You remove the plastic cap with a knife and use a flat tip screw driver to remove the screws and replace the seat.

the water level could be adjusted too high and it is running down the overflow tube. if the fill valve makes a noise trying to fill every 10 minutes then you have a flapper on the flush mechanism that is not sealing properly. it may be out of shape or have something stuck under it.

Try adjusting the float level. The float is what tells the toilet to stop filling. If the float is set too high, then water will just go down the overflow tube and never be able to get to the level it needs to turn off.
