I had the same problem with an HTC Aspire ( I think that's what it was called :} Anyway I formatted the micro SD card in fat 32 through my laptop using a micro sd to sd adapter and the sd card slot on my laptop. I then stuck the micro sd card back into the HTC and it worked. I heard formatting through another phone works too. Try it and let me know.
Can it be read in another device if so i would say it is password protected in you card menu in the phone will be a section to enter the password.
sd cards just seem unreadable if the password has not been given and it wont prompt
You will have to upgrade your dash to windows 6 if you want it to read higher compacity micro sd cards. This method i free and is the only way their is no unlock code to make the the software compatible, you must upgrade it. once upgraded you can use up to a 8gb micro sd, i have not tried a higher card than that.
The phone is designed to only hold a 2gb card. this is because a 4gb card uses high density memory the phone software can't read. There is a hack out there that I learned about at heliocity.net but it did not work for me.
