From what you stated ,,this could be a bad transmission or a shift sensor in the transmission ,,,this is a clutching problem with these automatics ..most cases have proven that you can change the transmissions fluid ,module and filter to allow proper shifting and the codes allow the clutching to return to normal...I am 90% sure from your post

Dear vdizon it seems your bearings are slipping inside your transmission.
This is usually caused by inproper use or stress to the transmission. Changing gears at high revs and double clutching may have caused the bearings to wear, or a service on the gearbox has not been performed by factory recomendation. I advise you to book your car into your mechanic to confirm the problem before the proble escalades.

If the clutch has never been changed most likely it is that. The way you can check it is by removing the transmission and looking at the clutch disc. It could be transmission syncros but I am willing to bet you just need a new clutch.
