Unlock codes come from the network provider - usually takes around a week after you call up and request it. I'd avoid other services like shops or websites that offer the service as we've had a phone bricked in the past when a shop did it. Regards the screen - that can't be a common or easy to fix problem so you need to take it to the shop.
