SOURCE: Installing a Leviton 5225 switch with outlet.
A switch is wired using the hot wires only, the black wire. It is simply a break in the line so that when the switch is off no power flows and when the switch is on the power does flow.
You are correct that the middle outlet would have two black wires and two white wires.
If you want to tap into that middle outlet and run power up the wall I suggest you look into some wiremold products. Go to their website, or any home depot and check it out. Get back if you think I can help you further. I am an electrician.
Here is a link that may help you.

SOURCE: wiring a leviton 5625-W switch receptacle combination
You cannot replace the switch with a switch/receptacle combo because there is no neutral wire. The black and white that you see are both used as hot wires. One is the Line and the other is the Load.
You have to install another 2 wire cable or replace the existing with a 3-wire cable. A 3-wire cable has a black, red, and white, plus the bare ground wire
SOURCE: I'm using a Leviton 1755 combo 3 switch for a bath
remove white switch one and connect to incoming white ground--all whites should be connected [hooked] together these are grounds-- switch 1 black from fan ,leave switch 2 red from fan ,switch 3 vanity black
SOURCE: how to install leviton 5225 wsp wall
For this combo , the white wires connect to the two silver screws on one side of the combo .The black power wire connects to the screw next to the switch. IF you want to have the receptacle hot all the time then jumper the hot black from the switch screw to the receptacle screw. The screw in the center is for the black wire from any lihghts that you want to control. IF it is the receptacle that you wish to control with this switch .. then the power connects to the screw next to the switch and the center screw jump a wire to the screw next to the receptacle
SOURCE: not sure how to wire this switch
Switch will have a commom terminal that the hot wire connects to.
The other three terminals are for the devices -- light, fan and other device.
The white wires should be the grounded wire returns for the circuits unless they are identified by color tape to be hot line side of power.
Be careful of the grounded side of circuit, can shock or kill, treat all wires as being energised. If you are not sure, use a electrical tester,
or get help.
