If your machine has a dial to adjust the stitch width, then make sure you are also turning this to the right setting so the needle "swings" as part of the stitch formation. However, if you are certain you have it set correctly, then other possibility is the needle bar bearings have siezed up or there is a spring inside the same area that pulls the needle back to centre, maybe its broken or come off. Either way you need to get inside the find out the answer; if you are not experienced in pulling apart a sewing machine, then I'd suggest its time for a trip to the SM technician for a service.

If you have put a new needle in then the chances are the machine has gone out of time, you can either take it in to the repair shop or you can contact me and I will attach timing instructions [email protected]
SOURCE: Zig Zag Stitch not working
THis problem occurred as I was sewing on Neoprene. I was using Gutterman all-purpose thread and a size 80 needle. Tried switching to a size 70, then to size 90. Still unable to zig zag. After inserting a test sample of cotton, the zig zag worked perfectly, so I knew it was not a mecahnical problem. I switched needles again. This time to size 90 for stretch fabric. Voila! Problem solved.
SOURCE: shark 384 - how can i get a straight stitch?
with the Shark Euro-Pro X machine, you need to bring the stitch width to zero it's the nob on top of the machine the manual states not to use it wider then 3 with the double needle cause it can break the needle. but either stitch selection "A" or "S" will give straight stitching with width at zero.
