My LG TV, model 32LB9D, spontaneously went to maximum volume and does not respond to any remote control: cable, TV etc. Even the volume buttons on TV do not work.
The fault may be on the control board. Sometimes the button switches can be stuck, or there may be something causing some conduction on the control circuit board. It is also possible to have a component failure that is causing a continuous command to keep the volume to the high position.
Something that causes failures of controls in TV sets is when people clean the screen, they spray liquid cleaner on the screen. Some of it drips down in to the controls and on to the circuit board. This will usually cause damage, and thus the set will require shop service.

I too had the problem with a Samsung Plasma 450. Its 10 yrs old but seemed to have fixed it by switching to the HDMI where there was no signal. Using the front panel controls I switched to the setting menu. Here the controls switched back and forth in rapidly scrolling through all the settings for TV mode. I managed to use the directional keys to slow down and then reverse direction of the selection. Flipped out of settings back to volume and sure enough it reversed and went the other way. Then i used the stereo for the TV sound. After a few days went back to the TV volume cntrl and is working... go figure.... you have to be patient and turn TV off and ON back to setting and the volume a few times before getting under control
