This manual is probably close enough. Your AX seems like it is for a certain world market whereas the rest has WPT. A4 paper is UK and Europe, we are in North America.

not sure its possible. tattiebogle will help assign the buttons. check under about this mac and locate the controller under the USB setting. if it is listed it is recognized. I set up a similar controller on 10.8.4
Error 1 means belt didn't start (or no speed sense).
Speed sensor and motor brushes are usually the culprit. Most of the time this its just the speed senor is out of place. Remove the hood and next to the pulley on the front roller you'll see a little black pill with wire attached. Adjust the tip of that to where its sitting within about an 1/8 inch from the magnet on the pulley. Speed sensor can go bad and you can tell by using a voltage meter on the audible continuity setting. New speed sensor: Cybex CX 445T Speed Sensor Part # AX 21500
If that doesn't work check your motor brushes, continuity on communication cables and calibrate it. If that doesn't work you may have a much larger problem such as a bad motor. How to test your motor
Did you try plugging it in to another USB port? This sounds like a problem with the USB cable of your controller or the USB port on your PC. Try bending the cable near the plug etc. and see if it makes any difference. If not you'll be better off buying a new controller unless its still under warranty.
