If your sim card has been blocked , then you need to get a new one ....

if your mobile phone is asking for a PUK code then you need to call up your service provider , give them your details and they should provide the PUK number to you.

Please call the Customer Care of your Network Operator (sim card provider), they will tell your the code. There's no other way or no means to get the code except from the Service Operator. They might ask your few questions related to the Sim Card, like your name or your address, tell them promptly and they will give you the puk code for your sim card. It's also important to note down the Sim Card Number (number written on the sim card) if in case they ask you the sim number. You will also be assisted on how to disable the security settings.

Note: if you keep entering random/unknown numbers/codes, the sim card will be locked and will be rejected. It'll not be usable anymore. Good luck.
