It's completely modem's problem, every modem has internal chip that's where it's store the configuration i think it is chip that is malfunctioning, change modem this problem will solve

Have you installed DSL filters on all your phones and make sure you don't have on on the line for your modem?
SOURCE: no internet when using router
Your modem has already registered itself with the MAC address of your computer, so it doesn't recognize the MAC of your router. In cases like this you have two options:
Clone MAC - Go to your routers Admin web interface and on the main page you should see the "Router MAC Address" section at the bottom. Select "Use Computer MAC Address" and click "Apply" and your router should reboot and you'll be back in business.
Soft Reset -
Plug the modem into the router via the ethernet cable then do a soft
reset.
To do a soft reset, do the following:

FYI for those people trying to use a ethernet and usb cable from a cable modem - it may work for a while but it won't work forever - your ISP will only allow one IP address out of the modem unless you pay for additional addresses (then you need a switch). Each device you have on the modem is fighting for that IP and eventually it will just quit working (if it works at all).
If you want more than one computer on your network (on the Internet or sharing printers/files, etc) then you need a router or a gateway (cable modem/router combo which I DON'T recommend).
