Try removing the splitter, if it works then you might have a faulty splitter and you need to get it replaced. You might check if you are using the first phone jack in your house / apartment. I remember that was key when i was on DSL. Hope you get it sorted out, not having a working connection is very annoying.
Earthlink is not your local telephone company? You would need to contact your local service provider. If earthlink set up your DSL thru your local telephone company, you may have to have them contact them. Does your local phone to you or do you subscribe thru what they call a CLEC?
You need to contact the network provider, the PUK is specific to your phone number and is related to the sim only. After a PIN has been enetered incorrectly 3 times it will request a PUK code which you will have 10 attempts. When contacting your network provider (o2, orange, t-mobile, etc.) they may ask security questions relating to your account.
