you can't format it by yoursely because you need a flashing device and nokia's software for that,
goto the mobile repair shop

You may try default phone and security code to remove restrictions.
ur defalt phones codes is 12345 or 00000 u can use it
Default 4 Digit user code: 1234 or 0000

type *#06# on your phone to get it.

Your MIC is having problem. Ucan just open the panel clean the MIC and check if problem continues change your MIC. It will solve your problem.
