Go to your local plumbing store and match up what you have.
that unit has circuit board problems. the OPT-3 box is not made anymore neither is the VTDS-12 control on the motor. You will need to locate a new control system with will probably have to have a new pump as well. You can locate replacement parts for your system at a couple of different sites.
