The lock button should lock the shaft. With the collet facing you, turn anti-clockwise with grips or pliers. They should never be done up _that_ tight. Try spraying with penetrating oil. Two version of the manual below.

It doesn't attach anywhere, that's why it popped out. It should just fit in a recess, pushing the lock button outward.

Remove the collet nut and collet then unscrew the sealing cap (black plastic ring), this has the same thread as the inner of the multi kit tool, so just screw on the tool then replace the collet and nut

The part that goes between the motor and the chuck is called the CLUTCH UNIT,
REF 2615294309 it is a splined rubberised sleeve, it also fits the 276 and 396.

I would go to Dremel.com or call their help line at 1-800-437-3635 and speak with one of their specialists.

I would send it back to Dremel, they have been AWESOME to do business with, I have sent in 2 of my rotary tools, and because they couldn't be fixed, they sent me new ones at no charge, also, just bought one of their 4" table saws (off ebay) that didn't come w/an owners manula, I emailed Drwemel, and got one in today's mail, pretty kool for an item they quit making years and years ago. Do you have a 'nut' on the end, or just a wide open shaft (1/3 inch or so?)
You need a collet nut to tighten bits, and maybe collets of varying sizes also (not sure about this particular shaft).

Unplug it, then while pushing in the little silver button near the head use a wrench on the hex nut and turn counter clockwise about a 1/2 turn. Push the bit in, then pull it out (that releases the little fingers that hold it).

The silver button locks the head so it won't spin while you are changing bits. Always be sure it is released before plugging in or turning on the power.
