These directions are for an AT and T phone, but other than that, are the same for any carrier. If you log onto your companys website under your account and go to the option that says devices you can get the puk code- If you enter in too many numbers it will permanently block your sim card and you will have to buy a new sim card from your cell phone company- they are about $20

Typically when a phone is asking for a PUK code it means you have tried to edit or set the secuirty pin code. Unfortunately these codes are already predefined by the sim card and the service provider. It gives you the sensation that you can set or change the code but you cannot. You get 3 attempts to enter the correct code after which the phone asks for a PUK code. When it asks for the PUK you also get three tries to undo it. Again PUK codes are predefined by the sim card and the service provider. If you call your service providers technical support number or customer service number they can provide you with the PUK or the PIN code.

The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
if you have entered the puk code wrong too many times you will make the sim card useless and you will have to contact your phone service provider for a new sim card, it will probably cost.
