The Constitution restricted the ablility of the Feds to tax and specifically forbids direct taxes like income tax, social security taxes and other payroll deductions. It was changed by Constitutional amendment. The promise was the payroll tax would never be more than 1% and would only apply to the very rich and would go away as soon as the cost of the Spanish American war was paid off.

The basic thing is that without the taxations of all types of your dues it could not be operated any more. It is also fact that Section 8 of the Constitution does, and the 16th amendment specifically authorizes the income tax. Federal Tax Levy is involving all around taxes and taxation departments transactions. Before that, income tariffs were the main source of revenues for the federal government. So anybody who tells you tariffs failed doesn't know history before 1913 and he did not have any kind of idea about failed of tariffs or anything.

What laws are congress exempt from?

Quite a few. They are exempt from the Affordable Care Act, they are exempt from SEC insider trading laws, they have exempted themselves from gun regulations in the past as well. Then of course there are quite a few Constitutional constraints on the Federal Government that Congress finds convenient to completely ignore. For that list, just read the Constitution; if it says the Federal Government cannot do it, you'll find that they are doing it.

Separation of powers that governs the US puts such local issues out of bounds of the federal government and outside the authority of the executive branch.
Attempts for federal control and "help" are unconstitutional and dangerous.

What taxes, if any, are included within a pro forma?

A pro-forma would usually include all the taxes that are payable by the vendor on behalf of the buyer. These are usually state sales taxes, or in some countries, may appear as a value-added tax (VAT) or goods & services tax (GST) applied as a percent markup of the transaction. The taxes included in a pro-forma should be those that apply to the transaction being taxed.

Is it a good? A service? A commodity, a major asset? Are there other fees & charges levied by local government that do not attract sales taxes?

The answer is:
It depends on what you are taxing, and in whose jurisdiction you are levying the tax and what rules apply to the calculation of the tax. Familiarise yourself with your applicable tax legislation.
