This forum is really designed for problems with the mechanics of sewing machines that are not working properly.
Your question seems to be asking about the construction technique of a particular Punjabi garment. 
Unfortunately it is very unlikely that you will find a sewing machine mechanic who also sews to the level you require as most only repair the machines, not work with them.

I am having trouble with pattern m4745 , civil war jacket. The back pleat instructions are confusing and there is not a good illustration. It seems like some part of the instructions are missing.

Sew the edges of the pleat, then lightly press open. Turn right side up.
Bring in the two sides together to meet the sewn seam, pin and press.
Sew across the top of pleat.

Look on YouTube under Box-Pleats the visual instructions will be understand. Good luck.

McCall pattern 4040 the instructions on how to form the box pleats. Is there a place that I can find clarification of where to start the pleats after sewing the skirt pieces together?

Usually the pattern has marks where each of the pleats should be made. (The pattern positioning of the pleats would change depending on the size being made.)

Sewing a hem on a sheer (rayon ?) curtain and it puckered slightly along the seamline Used sz 11 ballpoint needle with 40wt rayon thread. Tried tiny stitches then longer ones. Any suggestions??

well aside from trying all the normal things like thread tension adjustments and such , another neet little method is to use a flat piece of thin tissue paper (sew right thru it ) layed over fabric , this will add the stiffness to fabric u may need to complete this task, then very slow and carefull tear the paper away from it to leave the threads behind!! if you think you can get away with it use tissue paper on back side and leave it there , hey holds a hem and pleats real nice !!
