Unable to Read the Disc / Disc Could Not Be Read
Situation:
System displays either error message "Unable to read disc" or "Disc could not be read"
What to do:
This message may display if the disc is ejected during play. Return to the HOME Menu before changing discs.
If the Wii console is laying flat, verify that the system is not placed upside-down. The game disc slot should be above all three buttons and the SD Card slot.
Verify that the disc is being inserted properly.
If the console is placed vertically, the disc label will face to the right.
Note: Only Wii consoles with model number RVL-001 are intended to be placed vertically (learn more)
If the console is placed horizontally, the disc label will face upwards.

Situation Not Resolved:

What error messages that appear on your screen? If you see;
1 Disc read error or unable to reading disc, there is a problem with the laser lens, it can be happen if the lens is unclean, e.g. dusty, try to Cleaning the laser lens using disc cleaner. if you technical, try to make adjustment of the lens or replace the laser if necessary.

Call nintendo, if there is something in the USB port in the back of the system then take it out, it could cause freezing with out disc. If all of your disc are not playing ( Unable to read disc please refer to the wii operation manual ) its a problem with the disc drive in the system, set up a repair.

"Unable to read the disc" / "Disc could not be read" / "Eject the Game Disc" Error Message.
If you are getting one of the above error messages:
Ensure that you are properly loading a Game Disc.
If your console is placed vertically, the disc label will face to the right.
If your console is placed horizontally, the disc label will face upwards
If you know you are inserting the Game Disc correctly, but are still have the same problem with only one game (or if you only have one game), try cleaning the Game Disc by following the disc cleaning procedures

I had the same problem, I take it you have a modded wii(chipped), if you use a backup disc it is of a lower quality then original discs, the wii gets read errors without you noticing and tries to read those disc areas again sometimes suceeding and the gaming goes on, sometimes it fails, and a disc read error occurs.
Over time the dvd assembly will take some damage it will loose it rigidity it once had and the wii stops reading discs.
The problem gets worse if you have standing up, so first thing si to flip it down and try.
Otherwise you (or someone you trust) will have to open the wii and adjust the lens sled so it wont flap as much up and down.
