To know your PhilHealth Identification Number, type the following:

and its not working than Just visit any of their offices near your place then ask for it. If you were able to work in private/goverment companies, you can also ask their HR for your ID. So far, thats the easiest way to get it. There is another way and that is thru texting. See the source for details.
First you must have a SSS number because that is your temporary philhealth number, if you do have that one next thing is to go to your nearest philhealth branch then ask for an MDR then you will have your number.
Inform PhilHealth of your complete information like first name, middle name, last name, and birth date. You can reach the office through these contact numbers 637-3654 and 09189635396. You can reach the latter through text. You can also reach them via email ([email protected]) so you can verify your Member's Data Record (MDR).
