A PUK code is used to unblock your PIN incase a wrong PIN was wrongly entered five times, leading to your SIM card getting blocked.

Only your network service provider can provide you with a PIN and PUK for your SIM card.

Contact your network customer care service center and they will provide you with a PUK and PIN for free.

PUK code can be give only by your network provider which is specific for each and every mobile phone.
Contact your SIM customer care service department over the phone. It's free of cost to get the pin or PUK no. If you can't contact through your phone, get some other phone and contact them or go to the nearest service outlet of your service provider.
There is no other way than to follow the above procedure..

You need to get a PUK, PUKs, or personal unblocking keys, are used to unblock a mobile phone which becomes blocked. You entered your pincode wrong 3 times or more. You need your PUK code to undo. Your provider can provide you with this. There is NO otherway to unblock this (so called 'unlocking' is something else).

Complete information :
The SIM card protects your subscription and not the phone from unauthorised users. If you change SIM cards you can still use your phone with a new SIM card. Most SIM cards are blocked at the time of purchase. If your SIM card lock is on you have to enter your PIN (Personal Identity Card) every time you turn on the phone.

If you enter the wrong password three times in a row the SIM card gets blocked. This is indicated by the message PIN Blocked To unblock this you need to enter your PUK

card (Personal Unblocking Key). Your PIN and PUK is supplied by your service provider. You can edit your PIN and select a new four to eight digit number

• If the Message Codes do not match appears when you edit your PIN you entered the new PIN incorrectly.
• If the message Wrong PIN followed by Old PIN appears then it means that you have entered the old PIN incorrectly.
  

To unblock your SIM card:

1. When PIN Blocked appears enter your PUK > OK
2. Enter a new four to eight PIN > OK
3. Re-enter the new PIN to confirm > OK
   

To edit your PIN:

1. > Settings > the General tab > Security > Locks > SIM Protection > Change PIN
2. Enter your PIN > OK
3. Enter a new four to eight digit PIN to confirm > OK
   

To turn the SIM card On /Off

1. > Settings > the General tab > Security > Locks > SIM Protection > Protection and select On or Off
2. Enter your PIN > OK
