We have Inter-tel Axxess excecutive phones in my office. I have notices that sometimes the a would stop working (looks as if it were disconnected) and try the same phone at another jack and it will work. I try changing the cabling around, the phones, the ports, until somehow, I get them to work. They whole thing is very frustrating and I was wondering if there is a specific cable that has to be used, or why this happens???
Possible loose connection in the telcom room or room jack. Try re-punching all the connections that relate to that phone and test again. No special cables needed for exec. phone, just a single pair as any other.

When you say you try and 'change the cabling around' do you mean the phone cords? or the phone cables that go back to the telecom system?
