Cannot dial "3" on a Nortel CICS with 6.1 software
Anytime you dial a "3" from any phone on this Nortel CICS 6.1 system the call does not go through. I can change the line setting to "Pulse" and it works fine. I put my **** set directly on the CO line and I can dial 3 with no issue. It happens from any of the phones. I happens on all lines. I moved a few of the lines to another unused card in the system and still have the same issue. I replaced the fuses on the Incoming CO Line. I even bypassed the fuses to eliminate any possible issues.
Shy of replacing the cabinet and software I am at a loss.

Sounds like a programming error in the pbx software. I'd start with the dial plan and see if someone goofed with the number 3. It may just be corrupt. Happens on Nortel's all the time. They have to delete and rebuild the software regularly.

A Nortel Key Service Unit, an ICS (MICS,CICS), BCM (50,100 etc), Norstar (308,616,824), an Avaya IP Office running Nortel cards, or there is another company that makes a Nortel emulation key system but I can't recall what it is right now.

You have lost system programming. Unless you are well versed in getting into your system with the installer password, you need to call a Nortel service tech. The older systems have a battery backing up the memory and when it dies, your system crashes. It's really obvious, because it goes back to the default of only 2 lines working and all the buttons become defaulted.

It would be unlikely that your system is a CICS, but this is an idea of where the programs are and what level you need to access them.
Nortel M7000 series are proprietary sets which require a nortel MICS, CICS, or an older 8x24, 6x16, or 3x08 in order to work. If you have the system, it maybe that the jack isn't activated with a system port.
Try connecting 222 back up and look up the call forwarding cancel code. My guess, is your mystery phone system is a Nortel CICS which uses a FEATURE 4 to set and FEATURE #4 to cancel CF.
If you can't find the Call Forwarding codes in your user's guide, try posting back the system make and model number as a comment and I'll see if I can get you the proper codes.
The feature codes will not help you troubleshoot the system at all. But here you go:
