When shower running, the adjustable, vertically mounted (on a rod) mount/holder for hand held shower head slides downwards until spray only reaches as high as my waist. The fixture was installed 6 yeaqrs ago. There doesn't appear to be an easily reached adjustment. There appears to be a hole in the back of the "mount/fork" behind the hand held sprayer head that will admit only the smallest screwdriver (phillips-head or slotted) hex wrench but nothing will "engage."

you will need a vary small flat screw drive and at the bottom of the holder the is a small hole insert screw driver and un screw, it has a small screw that holds to the plate on the wall

I just got on myself. It is deceiving but it can adjust. You have to be very firm with it. I strongly recommend NOT having the GPS unit attached or having the mount on the window when adjusting as you have to bend it hard to move it. Just bend it whatever direction you want it to move and it will move. It is just VERY stiff. My guess is because that helps it maintain whatever position to bend it into. Good luck.

There should be a way to tighten the bolts without going that far Here's two things:
Look at the lid, with the seat down. The bolts are sometimes covered with a snap-in palstic cap or flap. If so, pry up with a screwdriver. Then use a screwdriver and wrench to tighten.
If the bolts appear to be cast as a part of the hinge assembly, you should be able to reach around the seat to the back side (no pun intended) of the toilet, and tighten with a wrench.
I sometimes use a 'mirror trick' to get a better idea of what's back there. Just use a small mirror and hold it to where you can see the bolt and not on the back.
